  1. Specific pieces of Personal Information we have collected about you;
  2. The categories of Personal Information we have collected you;
  3. The categories of sources from which we collected your Personal Information;
  4. The categories of your Personal Information that we sold or disclosed for a business purpose;
  5. The categories of third parties to whom your Personal Information was sold or disclosed for a business purpose; and
  6. The business or commercial purpose for collecting or selling Personal Information.
You can read more details about this in the Right to Know section of our California Privacy Policy.

The vast majority of information we collect is exempt from CCPA because, as explained in the Overview, CCPA does not apply to information governed by the federal Gramm-Leach-Bliley Act (GLBA) as well as other state and federal regulations like HIPAA. However, we will follow CCPA requirements to fulfill our obligations regarding Personal Information about a consumer that is not governed by GLBA or other exempted regulations.

You can submit a request to receive your Personal Information report from us by going to the Exercise My Rights page.

Please note that a Right to Know request must be verifiable. We will not share information about any individual without first verifying the identity of the person making the request and ensure that person is within their rights to make the request. We will also verify the identity of the subject of the request if that individual is not the requestor.
